We would like there to be an easy way for staff members to request for websites to be unblocked when they are redirected to the block page. Ideally, the workflow of it would be as follows:
1) Staff member encounters blocked screen and thinks it shouldn't be blocked
2) They click a button on that page and it automatically creates a request
3) The request sends a notification to organizational admins (by email ideally) that includes information such as requestor name, URL to unblock (taken from the last URL they attempted to reach?), and maybe even a reason that they could fill in.
4) We review the request and adjust the category of the URL as necessary
5) The requestor receives an email notification of the decision
